Sizemore has been very open about his struggles with substance abuse, he also appeared on the reality show Celebrity Rehab with Dr. Drew. The actor admitted he went into rehabilitation several times (via CNN):

I was fairly hopeless, and I wasn’t enjoying it, but I didn’t know how to stop. And having been to rehab many times and felt the pain of withdrawal and the horrible, for me it’s horrible, it’s been difficult first 90 days, 120 days, now I’m in an area that I haven’t been in before with the meth.”

Acute intoxication of drugs is considered a risk factor for brain aneurysm, which could have caused Tom Sizemore’s sudden collapse. The actor remains in critical condition.
